Candidates must possess an MS in Nursing or MPAS/MSPAS and hold a current unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in California. DEA registration or eligibility and CPR certification are required. For the Carelon Health business unit, a satisfactory Tuberculosis test completion is mandatory. Travel may be required. A Master of Physician Assistant Studies (MPAS) or Master of Science in Physician Assistant Studies (MSPAS) is highly preferred for PAs. Two years of experience in home-based services, complex care cases, and hospice or palliative care is preferred. Experience in bedside hospital, oncology, emergency room (ER), or intensive care unit (ICU) settings is highly preferred. Experience with Electronic Medical Records (EMR), specifically Epic, is preferred. Bilingual proficiency in English and Spanish is a plus.
The Field Advance Practice Palliative Care Provider will deliver palliative care services to patients in their homes, board and care facilities, and assisted living environments, utilizing both in-person and virtual methods. This role involves collaborating with physicians, other providers, patients, and families to develop and implement complex care plans tailored to the patient's goals, values, and health status. Responsibilities include conducting history and physical exams, ordering diagnostics, developing treatment plans, addressing physical and psychosocial needs, educating patients and families on medications and illness management, and coordinating care with other clinical team members. Providers will maintain patient medical records using Epic EMR, adhere to state regulations and company policies, and participate in continuing education. They will also prescribe medication as permitted by their licensing authority.
