Requirements

Availability for a limited-term position funded through a grant until 6/30/2026, contingent upon funding extension

Responsibilities

Provide case management to program participants, including researching and scheduling medical appointments, consulting with providers, training on prescription management, medical paperwork, record keeping, and transportation
Assist participants in applying for disability benefits, age-related SSI, and homemaker services as needed
Connect program participants to English as a Second Language (ESL) classes
Determine adequate referrals to Community Care Program units in Cook County and collaborate with assigned case-managers
Provide orientation to refugees, immigrants, and applicable family members about available social services and programs
Refer refugees and immigrants to external mainstream services and provide follow-up to ensure access
Conduct regular follow-up to ensure participants receive guaranteed services under the Older Americans Act and that needs are addressed
Facilitate enrichment groups covering topics like nutrition, navigating healthcare, mental health importance, and digital literacy to mitigate physical and mental health problems
Provide monthly opportunities for participation in local activity outings to increase community connections and reduce social isolation
Coordinate with World Relief staff, community organizations, and local churches to support vulnerable refugees and immigrants, including seniors
