Requirements

  • Registered Nurse (RN) licensure
  • Knowledge of Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) modality education programs, including hemodialysis (home and in-center), peritoneal dialysis, kidney transplantation, and hemodialysis access types (AV-Fistula, AV-Graft vs. central venous catheters)
  • Ability to hire, coach, counsel, and manage employees, including performance reviews, disciplinary action, and terminations
  • Leadership and development skills for direct reports, partnering with Human Resources
  • Proficiency in training Kidney Care Advocates on kidney education processes
  • Strong communication skills for routine interactions with physicians, partners, interdisciplinary teams, clinic managers, and external partners (e.g., NxStage, Azura, Interwell)
  • Knowledge of FMCNA/Fresenius Kidney Care (FKC) Home Therapies products, services, and strategies
  • Ability to maintain accurate and timely documentation in CTH database, Power BI, and/or electronic systems
  • Participation in FMS training and development opportunities

Responsibilities

  • Oversee CKD and ESRD modality education programs for a designated territory, ensuring comprehensive education for patients, caregivers, and support systems
  • Provide monthly reporting on Home Therapies referrals to Director of Operations, Area Team Leads, and/or Market Manager
  • Assist with patient navigation from Stage 4 CKD to dialysis initiation or transition to home therapy, including timely access placement
  • Maintain high-level communication with physicians, partners, interdisciplinary teams, and clinic managers
  • Support and implement referral processes with physician practices and clinics, evaluating effectiveness ongoing
  • Ensure timely responses to all modality education referrals
  • Assist in identifying, evaluating, and selecting Home Dialysis patients and Care Partners
  • Coordinate timely patient admissions, including urgent starts, into FKC Home Therapy and In-Center programs; collaborate to resolve barriers
  • Conduct Home Therapy awareness programs for patients and staff as needed
  • Meet growth goals by maintaining physician/partner relationships and increasing referrals
  • Collaborate on developing and implementing home therapy strategic plans with Kidney Care Advocate, RVP, Market Development, and HT DO
  • Promote FMCNA Home Therapies through community outreach programs
  • Provide educational programs to physicians as needed
  • Coordinate communication of corporate and business unit Home Therapies initiatives to physicians
  • Foster relationships with FKC partners to develop partnership opportunities
  • Cover both South Holland and Evergreen Park locations
