This role is critical in helping our members navigate the healthcare system more effectively and safely, empowering them to improve their chronic disease management, particularly diabetes. Certified Diabetes Care and Education Specialists (CDCES) are essential to this effort.

Role Responsibilities

The CDCES will manage a panel of members, primarily those with uncontrolled diabetes. You will guide members through their healthcare journey within our specialty clinic, aiming to help them achieve clinical stability and become better self-managers of their diabetes through education and counseling. This will be primarily delivered via virtual communication (phone and video visits).

Your responsibilities and impact will include:

• Screening members for Diabetes Program eligibility and appropriateness.
• Completing detailed assessments and providing ongoing follow-up with members in the Diabetes Program.
• Delivering education on all pillars of diabetes management, including disease process, medications, glucose monitoring, lifestyle counseling, and contingency planning.
• Scheduling follow-up appointments for members with yourself and other providers.
• Connecting members and providers with appropriate resources within Devoted Health to troubleshoot clinical issues.
• Collaborating with members to create and achieve SMART goals.
• Utilizing motivational interviewing skills to influence positive, sustained behavioral changes.
• Identifying gaps in care and providing relevant resources.
• Collaborating with other Diabetes Program team members (nurse practitioners, social workers, pharmacists) to develop comprehensive plans of care.

Required Skills and Experience

• Active, unrestricted Compact Registered Nurse license OR an unrestricted Registered Dietitian license.
• Active Certified Diabetes Care and Education Specialist (CDCES) certification.
• At least 2 years of experience in clinically managing diabetes.
• Ability to obtain additional state licenses as required by our operational footprint.

Desired Skills and Experience

• Remote experience managing diabetes is strongly preferred.
• Experience working with diabetes technology, including Continuous Glucose Monitors (CGMs) and insulin pumps.
