Requirements

Be trained and credentialed for all health plan clients and lines of business in the primary coverage state(s)
Maintain active state licenses in coverage areas
Maintain board certification (as applicable)
Maintain multi-state licenses (preferred)
Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale)
Flexibility to travel locally and within licensed states
May require hotel stay, car rental, and/or flight up to 20% annually

Responsibilities

Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area (majority of visits scheduled weekdays between 8 a.m. and 7 p.m.)
Provide 40 hours of availability weekly, consistently providing 160+ capacity units per month
Execute a strong visit completion rate, consistently pacing 110+ completed visits per month
Conduct Video Visits as needed (maintain training, affiliation, and compliance with Video Visit requirements, regular training required)
Maintain training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
Maintain all required supplies and associated diagnostic equipment related to DPS offerings
Conduct In-Home Health Evaluations (approximately 45-minute visits) including a medication and medical history review, a physical evaluation
Engage with regular manager, team and clinical meetings
Maintain accountability to Signify Health operational, clinical, and member experience key performance indicators and metrics
