- Bachelor's degree in Psychology, Social Work, Sociology, Education, Criminal Justice, Family Studies, Counseling, Recreational Therapy, Human Services, Human Development, Child Development, Gerontology, Behavioral Science, or Rehabilitation Counseling
- OR Associate of Applied Science in Behavioral Health Support degree from an approved institution
- OR Four years of qualifying work experience in a related human services field (e.g., case management, residential support, developmentally disabled support, vocational rehabilitation)
Responsibilities
- Provide individualized, community-based mental health services that empower clients to build skills, strengthen support systems, and progress toward personal goals
- Collaborate with clients, families, and support networks to develop and implement personalized treatment plans
- Deliver evidence-based interventions using approaches such as motivational interviewing, trauma-informed strategies, positive reinforcement, CBT- and DBT-based skills, and behavioral techniques
- Coordinate care and maintain strong partnerships with primary care providers, psychiatrists, schools, and community organizations
- Maintain accurate, timely documentation that reflects client progress and meets organizational standards
- Participate in ongoing professional development, training, and staff education activities
- Contribute to quality improvement processes through data collection, analysis, and reporting
- Foster strong, respectful working relationships with clients, families, teachers, counselors, and other professionals
- Prepare for and facilitate PSR group sessions, ensuring alignment with treatment plan goals
- Perform additional duties or responsibilities as assigned
- For ACT/ACT TAY/IMPART Only: Apply knowledge of ACT model principles, service processes, and core functions of ACT team operations
