Candidates should possess high levels of flexibility, strong communication skills, and an intimate knowledge of the community served, along with the ability to establish and maintain strong interpersonal relationships with patients, community organizations, and partners. A Bachelor’s degree is not required for this position, but experience working with patients and community resources is preferred.
The Community Health Worker will act as a bridge between patients, community, and medical systems to remove barriers and increase wellness, managing patient referrals, facilitating communication between involved parties, connecting patients to community resources, assisting with benefit applications, promoting goal setting, and providing patient-centered support in various locations, including patients’ homes and community settings.
