Candidates must hold a Bachelor’s degree in Nursing and be a Registered Nurse (RN) with at least 4 years of experience. They should possess clinical skills including IV insertion, IV/IM/SQ medication administration, 12 EKG, Phlebotomy, and other skills as permitted by local regulations. Prior experience in community care or home healthcare is preferred.
The Advanced Community Care Clinician will deliver medical care in the community, facilitating virtual provider visits and following medical orders, performing clinical skills within their scope, training, and policy. They will conduct various visit types such as ED at home, facilitated visits, outreach visits, specimen collection, and MIC follow-ups. The Clinician will also review equipment and communications, plan their day incorporating training and meetings, review patient information, conduct virtual visits, complete charting and documentation, process lab work, and ensure equipment is properly maintained and prepared for the next visit.
