  • Ability to ensure drug therapy appropriateness, dosing correctness, and cost-effective selection based on patient parameters
  • Proficiency in interpreting laboratory and patient-specific data to develop treatment plans and monitor therapy response
  • Experience issuing recommendations to medical teams based on medical literature and clinical experience
  • Ability to manage patients under collaborative practice agreements (where permissible), including initiation, adjustment, and discontinuation of medications
  • Skills in identifying and engaging high-risk or high-cost patients using population health tools, risk stratification, or EMR dashboards
  • Participation in quality improvement initiatives for chronic disease metrics (e.g., A1c control, blood pressure goals, medication adherence)
  • Ability to collaborate on reducing hospital readmissions and ED utilization
  • Capacity to develop and maintain clinical protocols, workflows, and documentation templates
  • Skills in tracking and reporting clinical outcomes, intervention metrics, and quality measures
  • Role as medication expert providing evidence-based recommendations for complex regimens
  • Effective communication of interventions and care plans via EMR and team huddles
  • Participation in care team meetings and provider education sessions

Responsibilities
