Participation in quality improvement initiatives for chronic disease metrics (e.g., A1c control, blood pressure goals, medication adherence)
Ability to collaborate on reducing hospital readmissions and ED utilization
Capacity to develop and maintain clinical protocols, workflows, and documentation templates
Skills in tracking and reporting clinical outcomes, intervention metrics, and quality measures
