Candidates must possess a Doctor of Physical Therapy, Doctor of Physiotherapy, or Master of Physical Therapy degree. A current, unrestricted PT license in Texas and Colorado is required, with a Compact License being strongly preferred. Bilingual proficiency in English and Spanish is essential, along with a willingness to obtain additional state licenses as needed. A minimum of 2 years of relevant experience in physical therapy or MSK-related clinical roles is necessary. Strong written and verbal communication skills tailored to both clinical and non-clinical audiences are expected, as is experience using data to optimize clinical workflows and identify opportunities for improved patient outcomes. Candidates should demonstrate the ability to navigate fast-paced, evolving environments with autonomy and accountability, and possess a high comfort level with technology, including Google Suite, CRMs, dual-monitor workflows, and real-time communication platforms.
The Clinical Navigator will conduct clinical intake assessments to evaluate patients' MSK conditions, identify goals, and determine treatment needs, building trust-based relationships using empathy, clinical insight, and motivational interviewing techniques. They will provide education, shared decision-making, and guidance to help patients understand treatment options and commit to evidence-based care plans. Responsibilities include monitoring patient progress, providing motivational coaching for therapy adherence, identifying and addressing barriers to care, and serving as a liaison between patients, care teams, and external providers to ensure continuity and coordination of care. The role also involves delivering timely clinical updates, escalating complex cases, partnering with care team members, providing peer support, and accurately documenting all patient interactions and assessments while adhering to established protocols for efficiency and productivity.
