Knowledge of insurance plans, medical terminology, and financial assistance programs is also required. Proficiency in databases such as Epic, Calibrio, CRM, Cisco phone system, and Physician database is necessary.
The Access Center Navigator will act as a point of contact for physicians, patients, and caregivers to assist with assessing Virtua Health services and converting interested callers into appointments or referrals. Responsibilities include navigating patients through the continuum of care, coordinating appointments, registering patients for programs, and providing physician referrals. The role also involves reducing gaps in care by identifying and helping patients overcome barriers, such as arranging transportation or connecting them with financial counseling. Additionally, the navigator will perform data entry, maintain confidential records, screen telephone calls, and deliver monthly reports on outcomes and performance.
