Requirements

  • Registered Nurse (RN) with entry-level designation into Clinical Advancement Program (CAP 2) or advancement from RN CAP 1
  • Meets RN CAP 2 criteria
  • Ability to perform physical demands: heavy lifting, moving patients, assisting with ambulation, frequent prolonged standing, bending, moving machines/equipment up to 200 lbs. (with assistance), lifting chemical/water solutions up to 30 lbs. as high as 5 feet
  • Completion of CAP requirements to maintain or advance
  • Knowledge of Fresenius Kidney Care policy, procedures, standards of nursing practice, state, and federal regulations

Responsibilities

  • Provision and coordination of clinically competent care: assessment, planning, intervention, evaluation for assigned patients
  • Delegation of appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians)
  • Ongoing, systematic collection and analysis of patient data pre-during-post hemodialysis treatment; document in medical record; adjust/modify treatment plan; notify Team Leader, Charge Nurse, Supervisor, or Physician as needed
  • Assess, collaborate, and document patient/family basic learning needs; provide initial and ongoing education
  • Direct and provide, in collaboration with staff, safe and effective delivery of hemodialysis therapy
  • Administer medications as prescribed or per approved algorithm(s); document justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as resource, leader, coach, mentor, role model for employees; set examples of behavior, work habits, attitudes
  • Ensure correct laboratory collection, processing, shipping; reschedule missed/insufficient collections
  • Identify expected outcomes; document/update nursing assessment and plan of care via Interdisciplinary Team collaboration
  • Ensure patient awareness of transplant and treatment modality options
  • Participate in education and quality improvement projects at facility and area level
  • May serve as Preceptor to new employees
  • Perform all other duties as assigned by Supervisor
  • Participate in decision-making, teaching, leadership functions, quality improvement activities to enhance patient outcomes and facility operations
