Requirements

Responsibilities

  • Conduct comprehensive survivorship assessments, including review of treatment history, potential late effects, and individualized risk factors
  • Develop and implement personalized survivorship care plans, including ongoing screening, surveillance, and preventive health recommendations based on national guidelines (e.g., NCCN, ASCO)
  • Support patients in managing chronic or late effects of cancer treatment, including fatigue, pain, lymphedema, cognitive changes, or endocrine disorders, in collaboration with oncology and primary care teams
  • Provide education and counseling to patients regarding lifestyle modifications, wellness, and risk reduction to promote long-term health and quality of life
  • Coordinate care between oncology, primary care, mental health, rehabilitation, and other specialty providers to ensure seamless continuity of care
  • Telemedically monitor patient progress and manage follow-up care via video, phone, and messaging to ensure ongoing engagement and adherence to care plans
  • Order and interpret laboratory tests, imaging, and surveillance studies as appropriate under medical supervision
  • Prescribe medications and supportive therapies within scope and according to established protocols
  • Document clinical encounters, assessments, and care plans accurately in the electronic health record, adhering to Color’s clinical policies and workflows
