Responsibilities

  • Lead the clinical team to provide the best patient care and a positive work environment, keeping patients happy, healthy, and out of the hospital
  • Practice primary care in the clinic alongside provider colleagues, dedicating 80% of time to clinical duties and 20% to administrative tasks
  • Serve as a critical leader by setting an example for thoughtful primary care, mentoring providers, and participating in strategic organizational initiatives
  • Collaborate with the Practice Manager and Outreach Manager/Director to achieve organizational objectives (Best Care Anywhere, Unmatched Patient Experience, Exceptional Growth, Great Place to Work & Leader in DE&I)
  • Drive implementation of OSH strategic initiatives
  • Cascade organizational information to care team members and escalate concerns/ideas to the organization
  • Coordinate and lead the onboarding of new providers at the center
  • Model excellent medical care, implementation of the care model, and success in metrics
  • Support consistent and robust OSH care model implementation through daily huddles, patient care planning, admission prevention, and clinical documentation workflows
  • Ensure appropriate, evidence-based care is provided by each provider and care team, keeping patients healthy and out of the hospital
  • Understand, teach, and implement Oak Street’s clinical guidelines
  • Support each care team and the center to provide the highest quality care
