Requirements

Bachelor's degree (Healthcare related or business field preferred)
1-3 years of Microsoft technical experience in Word, Excel, Outlook and ability to troubleshoot and resolve general technical difficulties
Ability to learn new systems and work in multiple platforms
Knowledge of and ability to navigate community and/or healthcare resources
Experience working with people who have SDOH challenges
Exceptional communication & interpersonal skills with ability to build rapport with internal and external members and stakeholders over the telephone
Strong written communication skills
Ability to multi-task and work in a fast-paced environment
Demonstrated understanding and respect of all cultures and demographic diversity
Ability to work remotely within Eastern or Central Standard Time Zone (typically Monday–Friday 8:00am–5:00pm EST)
Minimum internet speed of 25 Mbps download and 10 Mbps upload; work from a dedicated space lacking interruptions to protect PHI/HIPAA

Responsibilities

Using knowledge of social determinants of health (SDOH) to identify, research, document and coordinate services and resources to support member health and well-being
Guiding members and their families towards resources appropriate for their care and wellbeing
Serve as a key knowledge source for community services and information for members and other Humana associates
Providing education, contact information, and assistance in contacting basic resources such as transportation, food and housing
Work collaboratively with other Humana associates as a member of Humana’s Care Management Support team
Provide non-clinical support to the assessment and evaluation of members' needs and requirements
Contribute to administration of care management in coordination with RNs and Social Workers
