RN with current unrestricted Virginia state licensure
3 years clinical experience (med surg, behavioral health)
Associates Degree in Nursing (required; BSN preferred)
Must possess reliable transportation and be willing and able to travel up to 75% of the time (mileage reimbursed)
Work from home in VA with reliable phone and hard-wired internet service
Available Mon-Fri 8-5 PM EST; reachable via phone, videoconference, and email during work hours
Secure and private HIPAA-compliant workspace; personal responsibilities (e.g., dependent care) must not interfere
Proficient in multisystem navigation, using multiple monitors/systems simultaneously
Effective multitasker (talking and typing concurrently with focus and accuracy)
Basic technical troubleshooting (set up/connect hardware like monitors/keyboards, resolve common issues)
Experienced in electronic documentation and virtual collaboration tools
Highly organized, results-oriented, metric-driven, with clean/efficient workspace management

Responsibilities

Assessing, planning, implementing, and coordinating all case management activities with members
Evaluating the medical needs of members to facilitate overall wellness
Developing a proactive course of action to address issues and enhance short- and long-term outcomes
Enhancing member wellness through integration of services, strategies, policies, and programs (including network management and clinical coverage policies)
Working in field-based environments (company locations, client locations, facilities like nursing homes/rehab centers, member homes, community locations)
Performing close inspection of handwritten and computer-generated documents, PC monitor work
Sedentary work involving extended sitting, talking on telephone, typing on computer
Meeting productivity, quality expectations, and performance metrics
