Requirements

RN with current unrestricted Virginia state licensure required
3 years clinical experience (med surg, behavioral health)
Must reside in the vicinity of Pittsylvania and Campbell Counties, Virginia
Must possess reliable transportation and be willing and able to travel up to 75% of the time (mileage reimbursed per company policy)
Associates Degree in Nursing required (BSN preferred)
Work from home out of residence in VA with reliable phone and hard-wired internet service
Secure and private HIPAA-compliant workspace
Available Mon-Fri 8-5 PM EST, reachable via phone, videoconference, and email during work hours (8 hours per day)
Personal responsibilities, including dependent care, must not interfere with work duties
Proficient in multisystem navigation, using multiple monitors and systems simultaneously
Effective multitasker skilled at talking and typing concurrently
Basic technical troubleshooting for hardware setup and common issues
Experienced in electronic documentation and virtual collaboration tools
Highly organized, results-oriented, metric-driven performance
Clean and efficient workspace management
Preferred: Case management and discharge planning experience; Managed Care experience

Responsibilities

Assessing, planning, implementing, and coordinating all case management activities with members
Evaluating the medical needs of the member to facilitate the member’s overall wellness
Developing a proactive course of action to address issues presented to enhance short- and long-term outcomes
Enhancing a member’s overall wellness through integration of services, strategies, policies, and programs comprised of network management and clinical coverage policies
Working in field-based environments including company locations, client locations, facilities (nursing homes, rehabilitation centers), member homes, and community locations
Performing close inspection of handwritten and computer-generated documents as well as PC monitors
Engaging in sedentary work involving extended periods of sitting, talking on the telephone, listening, and typing on the computer
