RN/BH with current unrestricted Virginia state licensure required
3+ years clinical experience (med-surg, behavioral health)
Must reside in Stafford or southern neighboring areas
Must possess reliable transportation and be willing and able to travel up to 50-75% of the time (mileage reimbursed per company policy)
Virginia Driver's License required
Associates Degree required (Bachelor's of Science in Nursing preferred)
Independent Behavioral Health License in the state of VA in good standing
Reliable phone and hard-wired internet service
Proficient in multisystem navigation, using multiple monitors and systems simultaneously
Effective multitasker: skilled at talking and typing concurrently while managing responsibilities
Basic technical troubleshooting: able to set up hardware (monitors, keyboards) and resolve common issues
Experienced in electronic documentation and virtual collaboration

Responsibilities

Assessing, planning, implementing, and coordinating all case management activities with members
Evaluating the medical needs of the member to facilitate the member’s overall wellness
Developing a proactive course of action to address issues presented to enhance short- and long-term outcomes
Enhancing a member’s overall wellness through integration of services, strategies, policies, and programs comprised of network management and clinical coverage policies
Traveling up to 75% of the time to meet with members face-to-face
Working from home/field-based, reachable via phone, videoconference, and email during work hours (Mon-Fri 8-5)
Working in multiple locations including company locations, client locations, facilities (nursing homes, rehabilitation centers), member homes, and community locations
