Candidates must have a minimum of 6 months of experience in foster care, juvenile justice systems, adoption assistance, behavioral health services, trauma-informed care, ACEs, crisis intervention, or relevant evidence-based practices for Kentucky SKY populations. A Bachelor's degree is required, and preferred qualifications include 3+ years of experience in behavioral health, social services, social work, psychology, sociology, marriage and family therapy, or counseling, along with MCO experience. The role also requires reliable transportation and residency within the assigned Northern Bluegrass Region.
The Case Management Coordinator will utilize critical thinking and professional judgment to support the case management process, aiming to improve member healthcare outcomes through advocacy, collaboration, coordination, support, and education. This involves evaluating member benefit plans and available programs, identifying high-risk factors, implementing interventions, and coordinating wellness care plan activities. The role also entails consulting with case managers and other health professionals to overcome barriers, escalating quality of care issues, negotiating for necessary services, and utilizing motivational interviewing skills to promote member engagement and healthy lifestyle changes. Additionally, the coordinator will provide coaching to empower members in making informed healthcare decisions and ensure compliance with regulatory guidelines and company policies through case management and quality management processes.
