Candidates should possess a high school level education or equivalent, and a minimum of two years of clinical experience in a behavioral health inpatient, outpatient, or virtual setting, with experience in record management and requests. They must demonstrate a model of mood regulation, patient understanding, and empathy towards behavioral health patients.
The Social Services Care Team Assistant will engage with children in foster care who have complex medical and behavioral health needs, interacting with their caregivers, including placement staff, foster parents, and DFPS workers; they will help obtain updated medical consent forms when children transition to new placements or change caregivers; accept incoming phone calls and return calls from patients involving clinical inquiries, perform protocol-based triage, and route phone calls; place calls and create template-based letters regarding lab results; review schedules to ensure pre-visit preparations are complete and deliver necessary information to patients; call patients deemed to be at risk for a “no-show” and remind them of their appointments; initiate completion of forms for review and final signature by provider; assist care team in regular completion of reports and recognizing vulnerable patients, providing appropriate social service or community-based referrals; and perform other duties as assigned by the manager.
