Position Overview
We are seeking experienced Care Guides with a strong background in behavioral health coaching, care coordination, and safety/risk management for youth and their caregivers. In this role, you will empower families by teaching essential coping strategies, reducing challenging behaviors, and connecting them with appropriate behavioral health care and community resources.
Responsibilities
Assessment Review
- Thoroughly review assessment results from the onboarding process to understand member needs, challenges, and goals.
Risk Assessment and Safety Planning
- Refer members to crisis resources in cases of imminent risk (Note: Brightline does not provide immediate, 24/7 crisis services).
- Provide safety support for all members by conducting comprehensive risk assessments and creating safety plans with families during live or asynchronous communication.
- Ensure safety support for all members receiving services from Brightline (coach, therapist, or prescriber).
- Complete mandated reports and other documentation following legal processes and protocols.
- Re-evaluate members' fit for Brightline services (Coaching or Therapy) after a safety event.
Care Coordination and Referrals
- Refer families to relevant community resources, including support groups, local agencies, and social services (e.g., food, housing, financial assistance).
- Provide hands-on support to California families navigating services within or outside of Brightline.
- Facilitate internal referrals to Brightline Therapy services when appropriate.
- Coordinate external referrals to partner organizations such as schools, community health services, external therapists, primary care, EAP, and affiliate networks.
Coaching Members
- Provide evidence-based, culturally responsive behavioral health coaching to children, teens, and parents.
- Tailor care plans to align with family preferences, values, and cultural context.
- Deliver personalized coaching through video sessions and asynchronous chat using digital tools.
Documentation
- Maintain accurate and organized records of assessments, safety plans, referrals, and family interactions.
- Adhere to relevant privacy and compliance standards for all documentation.
Requirements
Minimum Qualifications
- Must be able to accommodate a 12 PM - 8 PM PST schedule.
- Option 1:
- Master's degree in a psychology-related field.
- 1 year of experience in a behavioral health setting.
- 2 years of formal or informal experience working with children, youth, or families.
- Option 2:
- Bachelor's degree and NBC-HWC or ICF certification.
- 5 years of relevant experience, including direct service in a behavioral health setting.
- 1 year of formal or informal experience working with children, youth, or families.
Preferred Qualifications
- Strong coaching skills, with experience in child and family-focused CBT, motivational interviewing, and SMART goal setting.
- Proficiency in SI, NSSI, HI, SUD, and ED risk assessments.
- Training and experience in conducting Safety Planning and Mandated Reporting are strongly preferred.
- A passion for learning and growth, with the ability to provide and receive feedback for effective collaboration.
