The Referral Coordinator will process, track, and coordinate patient referrals to ensure timely appointment scheduling, including PCP and specialist appointments. They will coordinate with pharmacies, verify patient PCP and insurance information (including prior authorization requirements), and manage the Release of Information process. Additionally, the role involves assisting with non-urgent patient follow-up and information collection, acting as a liaison between patients, providers, plans, and the internal team, and championing patient care continuity by navigating insurance and healthcare requirements.
