Pair Team cares for the highest-need Medicaid recipients through a community-led model. We build local partnerships with shelters, food pantries, and other community-based organizations to turn them into a site of care. As a support system for the community, we provide wraparound clinical services, up-skill CBO staff to become Community Health Workers, and utilize our proprietary data-driven technology platform, Arc, for care coordination. Through Medicaid MCOs, we provide healthcare for hard-to-reach, high-need individuals, while sharing healthcare dollars with community groups to expand their social support programs.

About the Opportunity

Pair Team is building a team of deeply passionate individuals ready to change primary care operations for those who need it most. We are looking for a highly motivated full-time Referral Coordinator who will serve as a crucial liaison between our patients, providers, Plans, and internal team.

As the Referral Coordinator, you will manage and facilitate the referral process, ensuring that our patients receive prompt communication and timely care. You will champion our patient care continuity by navigating insurance and health care requirements. You will work cross-functionally with the care team to ensure high patient advocacy while maintaining clear and efficient communication for all stakeholders involved.

This role is 100% remote; however, candidates must be based in the United States. Please note that we currently require all team members in this role to work PST hours.

What You’ll Do:

  • Own key Ops Support initiatives
  • Process, track, and coordinate patient referrals to ensure timely scheduling of appointments
  • Schedule PCP and / or specialist appointments for our patients
  • Coordinate with pharmacies (meds has been received, meds have been picked up, etc.)
  • Verify patient PCP and insurance (including prior auth requirements)
  • Coordinate Release of Information
  • Assist with non-urgent patient follow up and/or information collection

What You’ll Need:

  • 1+ years of backend healthcare and/or operations experience in a fast-paced healthcare environment providing administrative and operational support
  • Confidence to own your role and propose solutions to be successful
  • Bilingual - English/Spanish
  • A results-driven attitude with a proven track record of exceeding performance indicators
  • You are reliable and comfortable in an environment that is ever-changing to improve the wellbeing of underserved communities through increased access to high-quality care
  • Strong technical skills and comfort with utilizing multiple technology platforms
