Requirements

  • Full-time field-based telework position in Ohio, requiring ability to travel within Northeast Ohio region (up to 50% or more) to member homes and other locations
  • Must reside in Ohio; strongly prefers candidates residing in Mahoning or Trumbull counties
  • Clinically and culturally competent/responsive with training and experience necessary to manage complex cases in the community across child-serving systems
  • Monday-Friday 8-5pm schedule with flexibility to work later to meet member needs

Responsibilities

  • Conduct comprehensive evaluation of referred member's needs/eligibility using care management tools and data review; recommend approach to case resolution by evaluating benefit plan and available programs/services
  • Identify high risk factors and service needs impacting member outcomes; refer to clinical case management or crisis intervention as appropriate
  • Coordinate and implement assigned care plan activities; monitor care plan progress
  • Consult with case managers, supervisors, Medical Directors, and other programs using a holistic approach to overcome barriers; present cases at conferences for multidisciplinary review to achieve optimal outcomes
  • Work collaboratively with members' Child and Family Teams
  • Identify and escalate quality of care issues through established channels
  • Utilize negotiation skills to secure appropriate options and services meeting member's benefits and healthcare needs
  • Use influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes for optimum health
  • Provide coaching, information, and support to empower members to make independent medical and healthy lifestyle choices
  • Help members actively participate with providers in healthcare decision-making
  • Serve as a single point of contact for members; assist in remediating immediate and acute gaps in care and access
  • Utilize case management tools for monitoring, evaluation, and documentation of care
