Candidates must possess a Bachelor of Science in Nursing (BSN) and an active Compact RN license in good standing. A current California Nursing License is required, along with a willingness to become licensed in multiple states. Candidates should have at least 5 years of nursing experience, including 2 years in a Newborn/Pediatric setting, and must be eligible to sit for the CCM Certification exam in August 2025. Strong computer skills and the ability to navigate multiple medical record systems are essential, along with a commitment to following security and HIPAA regulations.
The Newborn/Pediatric Nurse Care Manager will deliver coordinated, patient-centered virtual care management via telephone and video to improve members' health outcomes. They will create impactful care plans with members and the care team, help navigate complex medical conditions and the healthcare system, and partner with local providers for coordinated care. The role includes providing compassionate follow-up care, assisting during acute healthcare episodes, and coordinating necessary resources to address clinical and social issues.
