Requirements

  • Bilingual in Spanish (Spanish speaking)
  • Willingness to conduct 1-2 on-site community visits per month near your city (position is primarily remote flexible, ~90% from home)
  • Highly motivated, creative, and empathetic thinker
  • Experience or ability to use evidence-based approaches and relationship-based strategies for engagement

Responsibilities

  • Maintain ongoing caseload of individuals through utilization of evidence-based approaches to promote engagement and achievement of health goals
  • Use relationship-based strategies to support members with social support navigation, understanding that many may have lived personal experiences causing them to (description cuts off)
  • Directly outreach and engage with individuals living with Serious Mental Illness/Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs
  • Participate in 1-2 on-site visits per month in the community alongside a fellow PairMate
