Bilingual in Spanish (Spanish speaking)
Willingness to conduct 1-2 on-site community visits per month near your city (position is primarily remote flexible, ~90% from home)
Highly motivated, creative, and empathetic thinker
Experience or ability to use evidence-based approaches and relationship-based strategies for engagement

Responsibilities

Maintain ongoing caseload of individuals through utilization of evidence-based approaches to promote engagement and achievement of health goals
Use relationship-based strategies to support members with social support navigation, understanding that many may have lived personal experiences causing them to (description cuts off)
Directly outreach and engage with individuals living with Serious Mental Illness/Substance Use Disorder, experiencing homelessness, and/or those who have high medical needs
Participate in 1-2 on-site visits per month in the community alongside a fellow PairMate
