Responsibilities

Conduct health risk assessments through calls to members to understand their health and needs fully
Coordinate with provider offices and home health agencies to ensure members receive necessary support
Check in with members over the phone to assess their well-being and confirm they have received the assistance they need
Actively participate in interdisciplinary rounds as an essential member of the care team
Review care plans for members to identify additional ways to provide support
Collaborate closely with other Devoted teams to address member needs effectively
Provide operational and administrative support to complex care department
Participate in SNP members' care plan operations workflows
Collaborate with other Devoted teams to address member needs effectively
Ensure members have the resources and support necessary to achieve better health outcomes
