Pair Team is on a mission to connect underserved communities to high quality care as a tech-enabled medical group focused on high-risk Medicaid and Duals patients. They have demonstrated an ability to engage a hard to reach patient population and address gaps in care, including a 56% reduction in ED visits and a 26% reduction in hospital admissions.

Responsibilities

  • Lead and supervise our team of Nurse Practitioners and Physicians, providing clinical oversight and mentorship to drive high-quality patient care.
  • Ensure Pair Team achieves its value-based cost-of-care and HEDIS targets by developing and executing new, evidence-based clinical protocols and policies in partnership with our Population Health team.
  • Serve as a key clinical leader, bridging clinical expertise with operational needs and collaborating closely with behavioral health, nursing and care management leadership to ensure seamless care delivery.
  • Partner with the Chief Medical Officer to inform clinical strategy and contribute to organizational growth.
  • Serve as the escalation point for complex clinical questions from our front-line teams.
  • Own and cultivate relationships with health plans, community organizations and other stakeholders, acting as a strategic partner to advance Pair Team's goals through impactful external engagement.
