Valid driver’s license, use of an insured vehicle, and an acceptable driving record

Responsibilities

Provides vocational training and employment opportunities to PACT consumers
Serves as an ongoing job coach to consumers
Collaborates with employers as necessary
Works as part of a team to provide education and support to PACT consumers as they navigate their wellness plans
Engages with participants in the community as they navigate social service, medical, and daily living needs
Completes all required documentation within programmatic timeframes
