Requirements

  • Graduate from an accredited medical college (within the United States/Canada) or from an unaccredited medical college (outside of the United States/Canada)
  • Board Certified in Emergency Medicine
  • 3+ years in clinical practice post-residency training
  • Experience in acute clinical care, managed care or value-based care operations
  • Experience providing comprehensive care management, chronic disease management, urgent home-based and community-based urgent care visits
  • Experience liaising with other providers around medical care, behavioral health, long term service and support needs, and the provision of palliative care
  • Has interest in working in a highly collaborative, integrated and multidisciplinary setting
  • Quiet private environment for virtual visits
  • Preferably 3 or more of the following state licenses: IL, IN, MI, OH, NY, MA, NC, FL
  • Part-time: 20 hours per week, with flexibility for 8-12 hour shifts, including rotating shifts (days, evenings, weekends, holidays)

Responsibilities

  • Delivers acute care for a complex population of members in a collaborative, interdisciplinary team model
  • Effectively supervises and provides leadership of clinical care within interdisciplinary team, ensuring highest quality of care
  • Ensures members receive appropriate care for acute and chronic diseases
  • Collaborates effectively with other Cityblock providers and longitudinal care teams
  • Prevents unnecessary emergency room visits
  • Timely documentation and loop closure
  • Active participation in Virtual Urgent Care and ED at Home model development, redesign, and innovation
  • Collaborates with and oversees advanced practice providers (NPs/PAs) during virtual visits
  • Works closely with Community Care Clinicians to plan next steps
  • Closes the loop with the member’s primary care team
  • Develops treatment plans and addresses acute needs via phone or video
  • Deploys Community Care Clinician ED at Home field team to implement assessment and treatment at the member’s home
