Requirements

Graduate from an accredited medical college (within the United States/Canada) or from an unaccredited medical college (outside of the United States/Canada)
Board Certified in Emergency Medicine
3+ years in clinical practice post-residency training
Experience in acute clinical care, managed care or value-based care operations
Experience providing comprehensive care management, chronic disease management, urgent home-based and community-based urgent care visits
Experience liaising with other providers around medical care, behavioral health, long term service and support needs, and the provision of palliative care
Has interest in working in a highly collaborative, integrated and multidisciplinary setting
Quiet private environment for virtual visits
Preferably 3 or more of the following state licenses: IL, IN, MI, OH, NY, MA, NC, FL
Part-time: 20 hours per week, with flexibility for 8-12 hour shifts, including rotating shifts (days, evenings, weekends, holidays)

Responsibilities

Delivers acute care for a complex population of members in a collaborative, interdisciplinary team model
Effectively supervises and provides leadership of clinical care within interdisciplinary team, ensuring highest quality of care
Ensures members receive appropriate care for acute and chronic diseases
Collaborates effectively with other Cityblock providers and longitudinal care teams
Prevents unnecessary emergency room visits
Timely documentation and loop closure
Active participation in Virtual Urgent Care and ED at Home model development, redesign, and innovation
Collaborates with and oversees advanced practice providers (NPs/PAs) during virtual visits
Works closely with Community Care Clinicians to plan next steps
Closes the loop with the member’s primary care team
Develops treatment plans and addresses acute needs via phone or video
Deploys Community Care Clinician ED at Home field team to implement assessment and treatment at the member’s home
