Registered Nurse (RN) with diverse healthcare systems background
Bilingual in English and Spanish
Expertise in building rapport with members/patients
Highly motivated, energetic, and focused on enhancing healthcare quality
Ability to work collaboratively with an interdisciplinary care team
Experience adopting new technologies to coordinate care and engage in shared decision-making
Comfortable leveraging data to observe trends and develop corrective action plans
Skills in assessing physical and psychological-social needs

Responsibilities

Deliver remote clinical guidance, support, education, and encouragement to members
Inspire behavior change and motivate members to adopt new behaviors for health improvement
Support the care team in identifying member goals, health priorities, and learning opportunities
Utilize motivational interviewing techniques
Design interventions with members to build engagement and achieve improved health outcomes
Assess physical and psychological-social needs
Coordinate care within a team pod structure, reporting to a nurse manager
