Responsibilities

Responsibilities include completing post-discharge questionnaires and calls, medication reconciliation, benefit education, monitoring members for changes in condition, and coordinating with providers and community resources to address health and social determinant needs. The role also involves managing warm transfers, evaluating health and social indicators, identifying barriers to optimal health, and facilitating care team coordination.
