Requirements

Responsibilities

Lead collaboration among interdisciplinary team to develop, coordinate, and facilitate direct social services interventions for assigned patients and support systems
Conduct individual, group, and family therapy sessions to address patients' needs and goals
Assess patients' mental health concerns, strengths, and challenges
Develop individualized treatment plans and guide implementation of clinical interventions, including facilitating treatment team meetings
Provide counseling and support using evidence-based techniques in sessions and milieu
Educate patients on mental health/substance use conditions, self-care, treatment engagement, and aftercare plans
Document patient progress, goals, and interventions in detailed records within required timeframes
Collaborate with treatment team members internally and externally for coordinated care
Report and document suspected cases of physical, sexual abuse, and/or neglect per state law and system policy
Participate in ongoing professional development and clinical initiatives for system-wide improvement
Demonstrate strong interdisciplinary collaboration and professional conduct
Communicate assertively and accurately, receiving feedback productively
Support staffing/scheduling changes as needed for program and patient care
Ensure compliance with Joint Commission, state, and regulatory standards
Provide for patient safety and facility safety
Perform other duties as assigned, including maintaining current knowledge of department processes
