Requirements

Responsibilities

  • Lead collaboration among interdisciplinary team to develop, coordinate, and facilitate direct social services interventions for assigned patients and support systems
  • Conduct individual, group, and family therapy sessions to address patients' needs and goals
  • Assess patients' mental health concerns, strengths, and challenges
  • Develop individualized treatment plans and guide implementation of clinical interventions, including facilitating treatment team meetings
  • Provide counseling and support using evidence-based techniques in sessions and milieu
  • Educate patients on mental health/substance use conditions, self-care, treatment engagement, and aftercare plans
  • Document patient progress, goals, and interventions in detailed records within required timeframes
  • Collaborate with treatment team members internally and externally for coordinated care
  • Report and document suspected cases of physical, sexual abuse, and/or neglect per state law and system policy
  • Participate in ongoing professional development and clinical initiatives for system-wide improvement
  • Demonstrate strong interdisciplinary collaboration and professional conduct
  • Communicate assertively and accurately, receiving feedback productively
  • Support staffing/scheduling changes as needed for program and patient care
  • Ensure compliance with Joint Commission, state, and regulatory standards
  • Provide for patient safety and facility safety
  • Perform other duties as assigned, including maintaining current knowledge of department processes
