Requirements

Registered Nurse (RN) licensure
Ability to work part-time (24 weekly hours, 2nd shift 3p-11p)
Comfortable working on-site at the Hospital at Home Command Center and making home visits (two visits per day or more as needed) in surrounding communities of Virtua’s five acute care hospitals
Ability to use leveraged technology and remote clinical care coordination
Willingness to travel (mileage provided)

Responsibilities

Perform and document the nursing process – assessment, diagnosis, planning, implementation, and evaluation of patients receiving care remotely or in-person at their defined home
Review and follow-up on patient education and implemented patient care plans
Assess patients, deliver medications, provide education, and perform other nursing interventions during home visits
Work collaboratively with a multi-disciplinary healthcare team to deliver therapeutic interventions within the patient’s home while maintaining high quality and safety standards
Access and document patient care
Bring essential elements of the inpatient care delivery model into the patient’s home using technology and remote coordination
