Requirements

Candidates must be a Graduate of an accredited school of nursing (R.N.) with 3+ years of experience. They should possess strong clinical exam skills and be proficient in conducting assessments, including behavioral health tools such as PHQ-9, GAD-7, AUDIT, or DAST-10.

Responsibilities

The RN Case Manager will coordinate with hospital case managers to understand members’ needs and complete discharge visits, conducting in-person or virtual assessments. They will also participate in daily inpatient rounds, conduct post-discharge follow-ups, and collaborate with care teams to develop care plans, ensuring member needs are met and barriers are addressed. Furthermore, the RNCM will perform medication reconciliation, track TOC-related metrics, and utilize electronic health records to document interactions and communicate effectively with the team and community resources.
