The Role

Pearl Health is seeking a skilled and dedicated Registered Nurse to join our team on a contract basis. This is a full-time (40 hours per week) remote position focused on Virtual Chronic Care Management.

In this role, you will be responsible for managing high- and rising-risk patients eligible for chronic care management across 2-5 practices. You will provide virtual care and support, manage complex patient care scenarios, engage with patients remotely, and utilize various Electronic Health Record (EHR) systems.

The estimated duration of this contract engagement is seven months, with the possibility of extension.

This position offers a unique opportunity to contribute to the evolution of virtual care models and the development of effective patient management strategies within value-based care, with a strong emphasis on population management and enhancing population health.

Key Responsibilities:

• Conduct initial and ongoing virtual assessments to understand patients' health goals, particularly during intake, and update assessments as needed.
• Provide comprehensive education to patients and their families regarding their conditions, treatment options, and self-management strategies to promote patient empowerment.
• Escalate patient needs to designated Clinical Champions at Pearl’s partner practices when appropriate, ensuring the practice facilitates necessary clinical care and coordinates with the patient and family.
• Perform medication reconciliation with patients.
• Educate patients on their prescribed medications, address any questions, and identify and resolve barriers to medication adherence.
• Monitor patient compliance with medication regimens through regular check-ins and document any ongoing chronic disease symptoms. Escalate to the Practice Clinical Champion if a re-evaluation of the medication regimen may be necessary.
• Develop and maintain a low-friction communication channel (e.g., via text message) for patients and families to use for questions or symptom exacerbation, utilizing various communication tools.

Requirements

• Registered Nurse (RN) qualification.
