Requirements

RN license and minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, or similar role (or equivalent combination of education and experience)
Current, unrestricted RN license in applicable state(s) (Indiana)
Reside in St. Joseph, Elkhart, Marshall, Kosciusko, Fulton, Cass, Miami, Howard, or Tipton County, Indiana (or within reasonable commuting distance)

Responsibilities

Perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination, and management of member's needs (physical health, behavioral health, social services, long-term services and supports)
Identify members at high risk for complications and coordinate care with the member and health care team
Manage members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of health benefits
Obtain thorough and accurate member history to develop individual care plans, establishing short- and long-term goals in collaboration with member, caregivers, family, natural supports, and physicians
Identify members who would benefit from alternative levels of care or waiver programs
Develop the care plan for member services, ensure access to those services, and assist with implementation by facilitating authorizations/referrals within benefits or extra-contractual arrangements
Interface with Medical Directors, Physician Advisors, and/or Inter-Disciplinary Teams on care management treatment plans
Assist in problem-solving with providers, claims, or service issues
Direct and/or supervise non-RN clinicians (e.g., LPN/LVN, LSW, LCSW, LMSW) by assigning tasks, verifying/interpreting member information, conducting additional assessments, developing/monitoring/evaluating/revising care plans, and reviewing performance
Provide overall management of member's case within scope of licensure, including supervision and direction to non-RN clinicians as required by the IN PathWays for Aging program, and optimize member health care across the care continuum
