Requirements

  • Knowledge of diet and kidney disease, including relationship to treatment process
  • Ability to assess patient learning needs and provide appropriate education
  • Proficiency in nutritional assessment and calculating diet prescriptions per standard of practice
  • Familiarity with FMCNA guidelines, policies, algorithms, and medical record documentation
  • Knowledge of Federal and state regulations and licensure guidelines
  • Understanding of nutrition-related lab parameters, adequacy measures (e.g., Kt/V, Urea Kinetic Modeling), fluid status, mineral metabolism, renal bone disease, anemia, iron status, and sodium/volume control
  • Ability to collaborate with interdisciplinary healthcare team, including on QEP, QAI, and process improvement activities
  • Skills in identifying malnutrition risks and developing interventions

Responsibilities

  • Assess patient knowledge of diet and kidney disease and provide tailored education
  • Deliver patient-specific education to patient/family/caregiver on nutritional status, adequacy, fluid status, and lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive patient assessments
  • Collaborate on developing written, individualized comprehensive plans of care
  • Calculate diet prescriptions according to standards of practice
  • Complete progress notes for chronic dialysis patients per FMCNA guidelines
  • Maintain accurate documentation in medical records per policies and regulations
  • Provide ongoing nutrition assessment and counseling to support effective nutritional status
  • Identify malnourished or at-risk patients and collaborate on interventions/resources
  • Review albumin levels and body weight monthly and implement plan of care with patient/family involvement
  • Monitor nutrition therapy adherence, adjust as needed, document barriers, and address goal achievement issues
  • Collaborate on Urea Kinetic Modeling (Kt/V), mineral metabolism, anemia, nutritional/psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Report nutrition QAI results and participate in interdisciplinary QAI program
  • Coordinate Pre-ESRD education and work with Case Manager on patient education/referrals for supplements
  • Review nutrition lab results, counsel patients/caregivers, recommend treatment changes, and communicate with physicians, staff, and external facilities
  • Participate in pertinent staff meetings
