Requirements

Active license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of practice
Practices under supervision of a registered nurse, advanced practice registered nurse, licensed physician, or other health care provider (for LPN)

Responsibilities

Provides resident care by reviewing physician prescriptions, communicating the nursing care plan and pertinent information to the health care team, coordinating with other providers, and providing physical, emotional, psychological, and spiritual support to residents, families, and friends
Develops nursing care plans by obtaining medical histories, examining residents, documenting physical condition, psychological status, and social factors, conferring with family and friends, developing nursing diagnoses, establishing care objectives, and defining interventions, schedules, and timetables
Teaches healthcare requirements by establishing resident care goals, instructing residents, families, and friends on conditions, medications, and self-care skills, answering questions, and providing information and resources
Evaluates resident care by adhering to therapeutic standards, measuring health outcomes, reviewing records, noting changing conditions and needs, re-evaluating care objectives, and modifying the nursing care plan
Maintains a safe, secure, and healthy resident care environment by adhering to infection-control policies, medication administration and storage procedures, controlled substance regulations, organization standards, policies, and legal regulations, and alerting others to potential concerns
Keeps resident care equipment operating by following operating instructions, troubleshooting breakdowns, maintaining supplies, performing preventive maintenance, and calling for repairs
Keeps resident care supplies ready by checking stock, anticipating needs, placing and expediting orders, verifying receipt, and stocking items
Coordinates admissions, discharges, and transfers to and from the facility
Directs, plans, and coordinates workflow of nursing assistants and other nursing personnel
