Requirements

Responsibilities

  • Manage the Hospital@Home department, ensuring high quality, safe, and efficient patient care
  • Plan, organize, and direct all department functions and activities; establish goals, objectives, standards of performance; develop policies and procedures
  • Be responsible and accountable for department operations in compliance with standards, goals, and objectives for optimal patient services between hospital and home
  • Demonstrate positive collaborative relationships with hospital and in-home departments, individuals, groups, health facilities, and referral sources
  • Establish operational systems to ensure maximum resource utilization and financial performance of the clinical continuum of care
  • Handle customer relations, including resolution of patient complaints, proactive communication with physicians, identification and referral of risk management concerns, and initiation of appropriate follow-up
  • Maintain current knowledge of clinical concepts, evidence-based practices, reimbursement issues, and applicable regulations
  • Assist with actions to achieve budget objectives and meet revenue and expense goals
  • Interview, hire, develop, train, and retain employees; plan, assign, and lead work; appraise performance; reward and coach employees; address complaints and resolve problems
