Requirements

  • Registered professional nurse (RN) with entry-level designation into the Clinical Advancement Program (CAP)
  • Ability to perform all essential functions under the direction of the Supervisor with guidance from the Educator, Preceptor, or in collaboration with another Registered Nurse
  • Capable of ongoing, systematic collection and analysis of dialysis data, documentation in patient medical record, and adjustments to treatment plans
  • Ability to assess, collaborate, and document patient/family learning needs for initial and ongoing education
  • Skilled in directing and providing safe and effective delivery of dialysis therapy in collaboration with patients, home care partners, and staff
  • Competent in administering medications per prescriptions or approved algorithms, with documentation of justification and effectiveness
  • Ability to initiate or assist with emergency response measures
  • Experience serving as a resource for the health care team and participating in staff training and orientation
  • Knowledge of correct laboratory collection, processing, and shipping procedures
  • Ability to identify expected outcomes, document nursing assessments, and update plans of care through Interdisciplinary Team collaboration
  • Capable of ensuring patient awareness of transplant and treatment modality options
  • Skilled in assessing home environments, identifying barriers, and evaluating readiness of Home Dialysis candidates and partners
  • Proficient in training Home Dialysis patients and partners on equipment operation and maintenance through a formal program
  • Required to complete CAP requirements to advance
  • Ability to handle physical demands: heavy lifting, moving equipment/patients up to 200 lbs. (with assistance), frequent prolonged standing, bending, and assisting with ambulation

Responsibilities

  • Provide and coordinate clinically competent care including assessment, planning, intervention, and evaluation for assigned patients
  • Assess and manage patients’ response to home dialysis training and treatment by following protocols and communicating issues to physicians
  • Participate in decision-making, teaching, leadership, and quality improvement activities as part of the ESKD health care team
  • Perform all duties in accordance with Fresenius Kidney Care policy, procedures, nursing standards, and state/federal regulations
  • Collect and analyze dialysis data, document in medical records, adjust treatment plans, and notify Supervisor or physician as needed
  • Assess, collaborate, and document basic learning needs; provide initial and ongoing education to patients and families
  • Direct and provide all aspects of safe and effective dialysis therapy delivery to assigned patients in collaboration with others
  • Administer medications as prescribed or per algorithms, documenting justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as a resource for the health care team; participate in staff training and orientation of new staff
  • Ensure correct laboratory collection, processing, shipping; reschedule missed or insufficient collections
  • Identify expected outcomes, document and update nursing assessments and plans of care via Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Assist in identifying, evaluating, selecting, and educating Home Dialysis candidates and Home Partners
  • Perform assessments of home environments and partner/family readiness for dialysis treatments
  • Train Home Dialysis patients and/or Home Partners on safe operation and maintenance of equipment and supplies through a formal program
  • Complete CAP requirements to advance
  • Perform all other duties as assigned by Supervisor
