Requirements

  • Prefer previous Dialysis Experience
  • Must be open to floating to other clinics within Chicago
  • Available for various shifts and work hours, mainly Monday-Friday, day hours
  • Ability to perform physical demands of the job (e.g., essential functions in clinic and home visit settings)
  • Complete CAP requirements to advance

Responsibilities

  • Empower, engage, and train dialysis patients on peritoneal and home hemodialysis using state-of-the-art training programs and equipment
  • Conduct initial home visits with another staff member present
  • Function as an onsite Patient Educator and Case Manager
  • Perform ongoing, systematic collection and analysis of dialysis data for assigned patients, document in medical record, adjust treatment plans, and notify Supervisor or physician as needed
  • Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education
  • Direct and provide, in collaboration with patient, home care partner, and staff, safe and effective delivery of dialysis therapy
  • Administer medications as prescribed or per approved algorithms, and document justification and effectiveness
  • Initiate or assist with emergency response measures
  • Serve as a resource for health care team, participate in staff training and orientation of new staff
  • Ensure correct laboratory collection, processing, shipping, and reschedule missed collections
  • Identify expected outcomes, document and update nursing assessment and plan of care through collaboration with Interdisciplinary Team
  • Ensure patient awareness of transplant and treatment modality options
  • Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners
  • Perform assessment of Home Dialysis candidate’s home environment, partner/family readiness, and barriers to home dialysis
  • Train Home Dialysis patients and/or Home Partners on safe, effective operation and maintenance of Home Dialysis equipment and supplies through organized training program
  • Perform all essential functions under direction of Supervisor with guidance from Educator, Preceptor, or in collaboration with another RN
  • Participate in decision-making, teaching, leadership functions, and quality improvement activities
  • Perform all other duties as assigned by Supervisor
