Requirements

  • Licensed as a Registered Nurse (RN, RN II, RN III) governed by the Nurse Practice Act, Administrative Code, professional standards, and Rogers Memorial Hospital policies
  • Ability to complete initial assessments, collect/record/analyze data on patient strengths, limitations, cultural/spiritual/ethnic factors, discharge needs, medical status, medication history, and age-specific needs
  • Skills in involving patient support systems (family, friends) in assessments and documentation
  • Knowledge of patient rights, advocacy, privacy, confidentiality, and obtaining consents/signatures
  • Familiarity with hospital policies on unit safety, gown/contraband searches, and patient orientation
  • Ability to initiate, update, and interpret treatment plans, participate in care conferences/staffings, evaluate data, review lab data, assess risks (falls, self-abuse, suicidality, assaultive behavior), and document changes
  • Competence in developing age-appropriate interventions and conferring with healthcare professionals

Responsibilities

  • Complete initial assessments and document as required, including patient strengths/limitations, cultural/spiritual/ethnic factors, discharge needs, medical/physical status, medication history, and age-specific data
  • Involve patient's support systems in assessment/documentation and observe/document interactions pertinent to treatment
  • Act as patient advocate: explain rights, obtain consents/signatures, protect privacy/confidentiality, assist in orientation
  • Employ hospital policies on unit safety, conduct gown/contraband searches respectfully, provide unit information sensitively
  • Initiate/update treatment plan: participate in planning/modifying care, evaluate data from multidisciplinary assessments, represent nursing in care conferences/staffings, write/review goals
  • Identify/interpret/document information from nursing interviews, observations, assessments, diagnostic data; review lab data and follow up with doctor
  • Evaluate risks (falls, self-abuse, suicidality, assaultive behavior) and initiate precautions/interventions
  • Assess changes in patient status, document interventions, and implement patient care, including safe/correct medication administration (right patient, etc.)
