Requirements
- Registered Nurse licensure
- Understanding of trauma-informed care and effects of trauma on health, coping, and lives of clients
- Ability to coordinate supportive and therapeutic behavioral health services for adults with severe mental illnesses, addiction issues, and criminal justice involvement
- Experience or knowledge in psychiatric principles, psycho-pharmacy standards, and medication management
- Ability to work Monday through Friday, 8:30 a.m. to 5:00 p.m., at Kerns building in NE Portland, OR
Responsibilities
- Provide quality nursing care, including all medical interventions and appropriate documentation
- Assist in formulating Nursing Care Plans for all medical/physical conditions
- Administer PRN medications based on presenting symptoms and proper assessment, in collaboration with the LMP
- Participate in planning, implementing, evaluating, and documenting direct client care activities and medication management
- Attend and actively participate in FACT Treatment Team meetings
- Act as a liaison with medical staff, pharmacy, laboratory services, and other outside agencies to assure continuity of care
- Administer psychotropic medications, interpret client response, and take appropriate action based on psycho-pharmacy standards
- Collaborate as a team member to establish medical treatment plans and client care
- Interact appropriately with clients, visitors, and community members; advise on client rights to treatment, refusal, and involuntary procedures
- Document and maintain electronic health record (EHR) and ancillary records in compliance with policies, procedures, and regulations; keep accurate documentation for billing, assessments, treatment, progress, etc
- Attend and participate in Nursing meetings and client case conferences
- Inform clients about medications and medical treatments; provide ongoing support/teaching on medication management for clients and families
- Call the LMP with laboratory results, clarifications of orders, changes in client status, or other relevant information
