Requirements

Candidates must be a graduate of an accredited school of nursing, possess a minimum of one (1) year of recent nursing experience as an RN or LVN/LPN (per state regulations), hold current licensure in the state, and maintain a CPR/BLS certificate. Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills as outlined in a competency checklist are also required, along with the ability to be a licensed driver with an insured automobile in good working order.

Responsibilities

The Registered Nurse (RN) – Home Health will assess patient and family needs to determine home care requirements, provide physical assessments and illness histories, initiate and revise care plans, develop care plans with patient and family involvement, identify discharge planning needs, and counsel patients and families on nursing and related needs. They will also monitor patient conditions and provide ongoing care, ensuring adherence to established care plans and collaborating with other healthcare professionals.
