Responsibilities

  • Assess patient’s knowledge of diet and kidney disease; provide tailored education on diet's relationship to kidney disease and treatment
  • Deliver patient-specific education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
  • Complete nutritional assessments and participate in interdisciplinary comprehensive assessments
  • Participate in developing written, individualized comprehensive plans of care addressing patient needs
  • Calculate diet prescriptions according to standards of practice
  • Complete progress notes on all chronic dialysis patients per FMCNA guidelines
  • Maintain complete and accurate documentation in medical records per FMCNA policies and regulations
  • Provide ongoing nutrition assessment and outcome-oriented counseling to help patients achieve effective nutritional status
  • Identify malnourished patients or those at risk; collaborate with interdisciplinary team on interventions
  • Review each patient’s albumin level and body weight at least monthly; implement plan of care with patient/family/caregiver involvement
  • Monitor adherence and response to nutrition therapy; adjust therapy, document barriers, and address issues to meet goals
  • Collaborate on Urea Kinetic Modeling recommendations to achieve physician’s Kt/V prescription
  • Collaborate on management of mineral metabolism, renal bone disease, anemia, iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
  • Adhere to FMCNA algorithm and medical record policies; report on nutrition QAI results and participate in QAI program
  • Collaborate on Pre-ESRD education and coordinate with Case Manager for patient education and physician referrals for nutritional supplements
  • Review nutrition-related lab results; counsel patient/caregiver; recommend and communicate treatment changes to interdisciplinary team, physician, facility staff, and nursing facilities
  • Participate in pertinent staff meetings
