Registered Dietitian certification
Knowledge of renal nutrition, diet, and kidney disease
Ability to perform nutritional assessments and calculate prescription diets
Experience providing patient counseling and detailed nutritional information
Adherence to FMCNA Compliance Program, regulatory, and FMS policy requirements
Ability to function as a member of an interdisciplinary healthcare team
Participation in Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) activities
Commitment to process improvement to help patients achieve individualized goals

Responsibilities

Provide nutritional services for in-center and home patients to maximize nutritional status and improve clinical outcomes
Assess patient's knowledge of diet and kidney disease
Offer patient-specific information about nutritional status, adequacy measures, fluid status, and nutrition-related lab parameters
Drive FMS culture through values and customer service standards
Deliver outstanding customer service to external and internal customers
Develop and maintain effective relationships through timely communication
Take initiative to respond, resolve, and follow up on customer service issues
Actively participate in process improvement activities
