Registered Dietitian credentials
Knowledge of diet and kidney disease relationship and its impact on treatment
Ability to assess patient's learning needs and provide appropriate education
Skills in nutritional assessment and calculating diet prescriptions according to standards of practice
Proficiency in completing progress notes and maintaining accurate documentation per FMCNA policies, Federal, and state regulations
Competence in monitoring lab results (e.g., albumin, body weight, nutrition-related parameters) and adjusting nutrition therapy
Ability to collaborate with interdisciplinary teams on Urea Kinetic Modeling, mineral metabolism, renal bone disease, anemia, iron status, nutritional status, psychosocial status, vascular access, modality, rehabilitation, and sodium/volume control
Adherence to FMCNA algorithms, QAI programs, and process improvement activities

Responsibilities

Assess patient’s knowledge of diet and kidney disease and provide tailored education to patient/family/caregiver on nutritional status, adequacy measures, fluid status, and lab parameters
Complete nutritional assessments and participate in interdisciplinary comprehensive assessments and development of individualized plans of care per Federal and state guidelines
Calculate diet prescriptions and provide ongoing nutrition assessment and counseling to achieve effective nutritional status
Identify malnourished or at-risk patients, collaborate on interventions, and review albumin levels and body weight monthly
Implement, monitor, and adjust nutrition therapy, documenting progress and reasons for unmet goals
Collaborate with interdisciplinary team on various clinical recommendations (e.g., Kt/V, mineral metabolism, anemia, etc.) and adhere to FMCNA policies
Report on nutrition QAI results, participate in QAI programs, and engage in process improvement activities
Coordinate Pre-ESRD education, work with Case Managers on care plans, and refer for nutritional supplements as needed
Review nutrition-related lab results, counsel patients/caregivers, recommend treatment changes, and communicate with physicians, staff, and external facilities
Participate in pertinent staff meetings (e.g., general staff, quality improvement)
