Responsibilities
- Provide behavioral healthcare management to patients, monitoring and managing care in alignment with a holistic approach using various treatment modalities as patients progress through recovery phases
- Co-manage a panel of complex patients through behavioral health assessments, diagnosis, treatment recommendations, treatment planning, clinical treatment, medication management, and follow-up planning
- Develop individualized treatment plans early in the engagement and stabilization phase, focused on addressing specific psychosocial and medical needs that might limit recovery
- Provide continuous, regular consultations to patients navigating recovery, both in person and occasionally virtually
- Liaise with patient care teams and collaborate closely with behavioral health providers to ensure an integrated, cohesive approach to care
- Address medical needs to mitigate negative impacts of various social determinants
- Partner with clinical staff, peer specialists, primary care providers, and community partners in a collaborative, shared decision-making model
- Act as a patient advocate, helping members navigate health and social services systems in Boston, addressing problems, answering questions, gathering and providing information and advice, and connecting patients to resources and the community
- Initiate, establish, and maintain relationships rooted in trust and rapport with patients and their families
