The Population Health and Wellness Care Management Coordinator will evaluate members through assessments and data review, recommending approaches to resolve care needs and maintain optimal health, identifying high-risk factors, and implementing proactive support interventions. They will coordinate wellness care plans, monitor member needs, consult with various healthcare professionals, identify quality of care issues, negotiate services, and utilize motivational interviewing skills to empower members to make healthy choices.
