Requirements

  • Serves as a practice-based provider supporting scheduled appointments, walk-ins, urgent/emergent visits across modalities (face-to-face, virtual, in-hub)
  • Delivers comprehensive chronic disease management (e.g., hypertension, diabetes, chronic kidney disease)
  • Manages evaluation of acute concerns
  • Manages behavioral health conditions (e.g., depression, anxiety, alcohol/substance use disorder)
  • Provides preventative care and wellness support (including sexual/reproductive health)
  • Works collaboratively with interdisciplinary team to develop member care plans and support members
  • Assists with development/enhancement of clinical programs, initiatives, processes, policies
  • Manages most complex patients in market
  • Remains up-to-date on evidence-based care for complex adult primary care patients (pediatric in certain markets)
  • Provides longitudinal direct primary care or wrap-around care focused on chronic condition management and preventative care

Responsibilities

  • Conducts comprehensive clinical intake visits for selected members (based on segmentation, empanelment, care team recommendation)
  • Conducts timely follow-ups post-transition of care, acute needs, or condition-specific needs
  • Proactively outreaches to members needing follow-up care
  • Provides clinical care per CBH protocols
  • Completes comprehensive diagnosis capture for HCC and ICD-10 coding
  • Addresses contractual/company-prioritized quality gaps with proper chart documentation/codes (ICD/CPT)
  • Addresses/responds to member needs and delegates tasks timely
  • Finishes documentation within 48 hours per company policy
  • Prescribes 1st/2nd line medications for depression, anxiety, alcohol/opioid use disorders
  • Continues psychiatric medications for stable SMI members
  • Refers members to specialist care per internal CBH guidance
  • Supports members in achieving care plan goals (physical, behavioral, social health)
  • Participates in case conferences providing clinical expertise for care plan development
  • Provides guidance on highest impact clinical interventions/tasks
  • Collaborates on member panel for clinical input (health maintenance, chronic disease, psychiatric support)
  • Collaborates with team on member program placement (e.g., lower/higher intensity)
  • Utilizes care facilitation, EHR, scheduling platforms for data collection, documentation, task tracking, communication
  • Educates/supports/elevates team members' skills (informal coaching)
  • Acts as collaborating physician for Advanced Practice Clinicians oversight
  • Completes chart reviews per CBH policy
  • Guides clinical team on complex cases, available for escalations, provides clinical consultation (issues, care plans, concerning conditions)
